Support Planner

Digital Connect, based in Mansfield, is offering a permanent position for a Support Planner, working 15 hours per week at Grade 3 core, with a salary of £25,235. Operating across the seven boroughs of Nottinghamshire, including Ashfield and Rushcliffe, the service focuses on assisting individuals referred by Adult Social Care.
