98Job Description:We have an exciting opportunity to join our Healthcare team who provide a range of healthcare to vulnerable children and young people resident in the home, aged between 10 and 18 who have often experienced many adverse childhood events and trauma.
The team enhances this care by providing multi-disciplinary support for healthcare needs, including, physical health, mental health, and psychological/emotional well-being. You will carry out assessments, developing therapeutic groups and working 1-1 with young people and will manage a defined caseload of young people with occupational needs, using evidence based, client centred, recovery, health promotion and relapse prevention orientated principles and practice.
You will also provide professional advice and support to other agencies and members of the multi-disciplinary team supporting resettlement and transitions. You will work in partnership with your health team and the unit care staff and contribute to the health screening of young people on admission.
The core hours are 9.00am to 21.00pm Monday-Friday with some on-call for initial health screening of admissions up until 13.00pm weekends and bank holidays. The team are committed to professional development providing many training opportunities for staff. Flexible working or job share can be supported. This position may become permanent in the future, depending on the funding.

To deliver care planning and individual and group interventions, in partnership with young people and other workers involved in care delivery.
To lead on sensory assessments that will inform care plans for individual children and young people.
Work in partnership with the resettlement worker and support discharge planning and transition.
To proactively involve young people and families/carers in service development and evaluation.
To provide a professional, non-stigmatising service to children and families/carers up to 18 years.
Work collaboratively, and promote effective working relationships, with members of the multi-disciplinary team, ensuring effective clinical decision-making, internally and with external agencies.

Provide OT specific assessment and interventions to individuals under the care of the inpatient ward.
Establish therapeutic relationships with patients; implement evidence based interventions in accordance with professional code of conduct, performance, ethics and standards of proficiency of registrant body (HCPC).
Recommend adaptations to a patient’s physical and social environment assess for falls hazards and prescribe equipment where appropriate.
Maintain the welfare and safety of patients and protect them from any form of abuse in accordance with safeguarding guidance.

To work flexibly between 9am and 9pm, including some on-call provision at bank holidays and weekends.
To work in accordance with relevant childcare and health legislation and professional standards.
To proactively involve young people and families/carers in service development and evaluation.
Participate in programme of health promotion.
To function as part of the multidisciplinary LSU service.
To work in partnership with wider CAMHS, Children’s Services and Youth Offending Service.
To provide a professional, non-stigmatising service to children and families/carers up to 18 years.
Work collaboratively, and promote effective working relationships, with members of the multi-disciplinary team, ensuring effective clinical decision-making, internally and with external agencies.
Empower Service Users and their families/carers to influence and use available services, information and skills and act as an advocate where appropriate.
Participate in the planning and delivery of group interventions
Adhere to Trust Policies in consideration of Children in Need and safeguarding issues and in maintaining accurate and effective written records.Location: Sleaford
