Are you an experienced and skilled Advanced Nurse Practitioner seeking an exciting opportunity? Join the dedicated team at a reputable hospice in Ulverston and make a significant impact to their patients’ lives!

Location: Ulverston Reporting to: Head of Clinical Services

Key Responsibilities:

  • Work autonomously as part of the medical team, demonstrating exceptional clinical expertise.
  • Conduct comprehensive patient assessments and provide high-quality care.
  • Utilize a range of assessment methods and advanced health assessment skills.
  • Act as an Independent and Supplementary Prescriber, ensuring safe and effective medication management.
  • Undertake advance care planning and promote patient-centered care across all hospice services.
  • Lead key initiatives, foster collaborative practices, and improve patient care and outcomes.
  • Engage with multidisciplinary teams, both internal and external, to enhance collaboration and knowledge sharing.
  • Continually assess and monitor risk, champion evidence-based practice, and participate in research and evaluation.
  • Foster your own professional development and support the growth of nursing and care staff.

Requirements:

  • Advanced Nurse Practitioner qualification and relevant certifications.
  • Extensive experience in clinical practice and advanced nursing assessments.
  • Proven leadership skills and the ability to drive change and innovation.
  • Excellent communication, negotiation, and influencing abilities.
  • Demonstrated commitment to quality improvement and ongoing professional development.
