Job Summary by the Career Balance Team:

A Primary Care Network (PCN) in Leatherhead and Bookham is seeking an experienced Advanced Nurse Practitioner to join their team. The successful candidate will work autonomously to provide high-quality, patient-centred care, adhering to practice guidelines and protocols. Responsibilities include triaging and treating patients, prescribing medication, and conducting health screenings. The role requires a Registered General Nurse with a Master’s degree and current NMC registration. Additional qualifications, such as independent prescribing and teaching certification, are desirable. The position is full-time or part-time, with a salary dependent on experience and benefits including additional leave and a pension.

Title: PCN Advanced Nurse Practitioner

Responsible to: Primary Care Network Directors and Operations Manager

Location: Various, within Leatherhead and Bookham

Hours per week: Up to 37.5 hrs per week

Salary: Dependent on experience

Holiday Entitlement: 27 days per year plus Bank Holidays

We are looking for an enthusiastic, experienced and caring Advanced Nurse Practitioner, with excellent clinical skills and warmth, with a commitment to positively contributing to our provision of high quality patient centred care.

Our PCN incorporates four practices in Leatherhead and Bookham. The successful candidate will be working over one or two sites and will see patients in those practices.

JOB SUMMARY:

  • To work autonomously, providing a high quality, safe and supportive environment for patients, meeting their identified physical and psycho-social needs within the scope of individual accountable practice
  • Work within Practice guidelines, policies, protocols and procedures reporting outcomes to appropriate clinical staff so standards of care can be monitored and improved where necessary. This includes adhering to the following agendas: infection control, equality and diversity, health and safety
  • Maintain effective communication with patients, carers, relatives and members of the multidisciplinary team ensuring information is shared appropriately and confidentially so patient needs are met in accordance with NHS directives and safeguarding policies (DH 2003; DH 2010)
  • Protect and maintain patient records to the agreed standard and in accordance with professional record keeping standards (NMC 2009)
  • Administer medicines in accordance with a non-medical prescribing framework, patient specific direction or patient group direction in accordance with the standards for medicine management (NMC 2007)
  • Organise, manage and prioritise own workload ensuring care of patients is paramount
  • Develop the role by using evidence-based practice and continuously improving own knowledge as defined in the PREP handbook (NMC 2011) and revalidation guidelines

KEY RESPONSIBILITIES/DUTIES:

  • Triage and treat patients presenting with minor illness and wishing to see a healthcare professional; either in surgery in in their own home; making any necessary referrals to other members of the primary healthcare team
  • Advise patients on general healthcare and minor ailments, with referral to other members of the primary or secondary healthcare team, as necessary
  • Prescribe medicines in accordance with NMC Standards for Medicines Management (2007), NMC Code of Professional Standards (2015) and the Nurse Prescribing Implementation Plan (1998)
  • Ensure clinical practice is safe and effective and remains within the boundaries of competence and to acknowledge limitations
  • Minor Illness / Minor Injury Clinics
  • Chronic disease and admissions avoidance reviews and care planning
  • Palliative and End of Life Care and completion of DNACPR/Respect forms as required
  • Medication Reviews
  • Falls, Mental Health/Capacity, Social Needs Assessments
  • Health screening, contraception, dietary advice, blood pressure checks
  • Perform or request blood tests, urinalysis, ECGs (if required)
  • Cytology, Injections, Dressings, Removal of sutures/clips (if required)
  • Provide general and specific health screening to the practice population, within agreed protocols, with referral to other members of the primary and secondary healthcare team as necessary

The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the practice Health & Safety Policy, to include:

  • Using personal security systems within the workplace according to practice guidelines
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
  • Making effective use of training to update knowledge and skills
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
  • Reporting potential risks identified

  • Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
  • Taking responsibility for own development, learning and performance and demonstrating skill and activities to others who are undertaking similar work
  • Training may need to be undertaken outside of normal practice hours, and off site

Person Specification

Qualifications/Education:

ESSENTIAL:

Registered General Nurse with evidence of current NMC registration

Educated to Master’s degree as detailed within the HEE document tilted Multi-professional framework for advanced clinical practice in England

DESIRABLE:

Qualified independent prescriber – V300

Teaching qualification

ALS and PALS

Experience:

ESSENTIAL:

Experience of practice within the four pillars

Job plan that demonstrates advanced clinical practice and has equity with peers working at this level

DESIRABLE:

Experience of prescribing and undertaking medication reviews

Experience of working in a primary care environment

Clinical Knowledge & Skills:
