Are you passionate about helping older people live safely and confidently at home? We’re looking for someone caring, proactive, and motivated to join our team and make a real difference.
In this role, you’ll be a friendly face and a trusted support for older people as they leave hospital. You’ll help make the journey home smooth and safe, reduce the risk of falls or readmission, and ensure people feel confident and supported in their own surroundings.
This is your chance to work alongside hospital staff, community teams, and voluntary sector partners — bringing everyone together to create the very best outcomes for patients and their families.
What you’ll do:
- Support safe, timely hospital discharges and home visits
- Identify risks, recommend aids, and escalate concerns when needed
- Connect people with community and voluntary services
- Keep clear records and ensure every discharge is inclusive and person-centred
About You:
We’d love to hear from you if you are:
- Experienced in working with older people in health, social care, or community settings
- Confident in assessing needs and spotting risks in the home environment
- A strong communicator who enjoys working with patients, families, and professionals
- Organised and comfortable with record-keeping and reporting systems
