*Reablement Support Worker – Alternate Weekends*
Salary- £12.75 per hour
*Job Description:* An exciting opportunity has become available to join our team at Libertas! We are looking for dedicated, enthusiastic people who enjoy supporting people in the community.
As a Reablement Support Worker your day would consist of visiting Clients in their homes, ensuring safety and comfort, supporting with day-to-day activities and encouraging independence where possible.
Reablement is to encourage the service user to be as independent as possible and to follow personalised support plans in order the achieve goals set by the service user. Libertas looks for an approach that, irrespective of diagnosis we can find new ways for an individual to continue living as they wish, for Reablement *practise makes progress! *
*Responsibilities would include but are not limited to:*
* Supporting client’s post-hospital stay, fostering independence and dignity
* Conducting visual risk assessments and prioritizing client safety
* Build trusting relationships and aiding clients in goal achievement through personalized action plans
* Ensure clients understand their rights and service-related policies
* Maintain a high service standard and collaborate with the Registered Manager and partner agencies
* Assist with daily tasks and personal care
*Shifts* Full time/Part time
*All positions/shifts are to include every other weekend to give a good work life balance. *
Additional Compensation:
* Travel and Mileage paid separately to salary
* Car Mileage Allowance: 30p per mile
* MOT paid
