Responsibilities:
– Be the first point of contact for patients and provide high-quality patient care in-home, ensuring the comfort and safety of those under your care
– Administer medications accurately and in accordance with NMC guidelines.
– Prepare and maintain patient records including care plans accurately and effectively, and ensuring legal requirements are complied with in an ethical manner
– Assess, understand and monitor patients’ welfare by discussing their conditions with them and providing access to therapeutic activities
– Conduct risk assessments and monitor progress with the family
